Information for foreign nationals attending
this hospital for the first time

Please be advised that in cases where urgent communication is required in a foreign language at this hospital, tablet devices may be used for daily conversation, which may require some time in correspondence.   • As a general rule, a referral is required for the first visit at this hospital. If you are attending this hospital for the first time, you will need to obtain a letter of referral from another medical institution (treating doctor).
  • After the symptoms have settled from treatment at this hospital, we recommend that you see your treating doctor or another medical institution.
  • For consultations, the patients with referrals are seen first. If you have a letter of referral from another insurance medical care institution, we recommend making an appointment at the reservation center for referred patients beforehand to shorten the waiting time for administrative processes.
  • Please be advised that while we always make the best effort to see you as soon as possible, the patients without an appointment may be required to wait for a long period of time.

Health insurance certificate and patient card

In the Japanese medical care insurance system, medical remuneration points are assigned to each type of medical care performed, such as diagnosis, treatment, consultation fee, and hospitalization fee, and the unit price of each remuneration point is calculated as 10 yen. The expense for medical care is calculated from the total price calculated, and you will be asked to pay the out-of-pocket expense at the ratio specified according to the type of insurance you currently carry. If you have an insurance certificate, you will be paying 10% to 30% of the medical expenses; however, the entire medical expense will need to be paid if you do not have an insurance certificate.

  • If you are staying in Japan for a short period of time (less than three months) on holiday or business trip and do not have a Japanese health insurance certificate
    -› Please bring your passport and documents that show the membership number of the traveler’s insurance and medical insurance you are covered by to this hospital.
  • If you are staying in Japan for a long period of time (three months or longer) for study and work and are covered by Japanese health insurance
    -› Please bring your passport, residency card, and health insurance certificate to this hospital.
  • If you have unpaid insurance premiums, this may cause problems in the payment of medical expenses. Please be sure to check that there are no overdue payments.
Confirmation of the insurance certificate
  • Please be sure to bring the insurance certificate to the first visit.
  • If you are attending this hospital for a return visit, please be sure to bring the insurance certificate for your first visit of the month. If you are unable to present your certificate, you will be required to pay the same amount as those without an insurance certificate.
  • If your insurance certificate is renewed or if the details are changed, please present your certificate to Reception No. 4 on 1F (payment reception).
If you have forgotten to bring your insurance certificate
  • You will be required to pay the full amount if you have forgotten your insurance certificate.
  • The medical expense will be recalculated and refunded if you bring your insurance certificate to Reception No. 4 on 1F (payment reception) by the end of the month after the month of the consultation. A refund will not be given at hospital reception after this date.
Patient card
  • Please be sure to bring your patient registration card to your visit
  • The card will be reissued if you misplace or forget to bring the card (with the handling fee of 220 yen). Please inquire at Reception No. 1 on 1F (general reception).
  • The same patient card is used for all departments.

If you do not have a referral from another medical institution

  • If you attend this hospital for a first visit without a letter of referral from another insurance medical institution, you will be required to pay 7,700 yen (tax included) in addition to the out-of-pocket expenses for insurance medical care. (this fee will not be covered by your health insurance)
  • Also, if you are referred to another medical institution by this hospital and attended this hospital again without a letter of referral, you will be required to pay 3,300 yen (tax included) for each visit.
  • A letter of referral from a medical institution (treating doctor) is mandatory for consultations at the Department of Orthopedics, Dermatology and Obstetrics and Gynecology.
  • Consultations are not available without a referral unless urgent medical care is considered necessary.

Method of application for medical certificates

Fill in the necessary items in the application for medical certificates
and submit this to Reception No.2 (document reception) on 1F. 

  • Application is not accepted via phone, post, or e-mail.
  • If you already have a form for a medical diagnosis, fill in the address, name, sex, date of birth, age, and other information on this form and submit the completed form.
    [Remark] You may be handed a medical certificate prepared at this hospital using a different form than what you have.
  • Medical certificates must be applied for by each medical department.
  • If you wish to receive the medical certificate by post, you will need to pay the postal expense (434 yen, or 470 yen if five pages or more of documents are posted) in addition to the expense of the medical certificate. We do not post the medical certificate for liability purposes.
  • A power of attorney letter is required if the application is being submitted by those other than the patients or their family members living in the same household.
Location of submission

Please submit the application at Reception No.2 (document reception) on 1F.

Payment method
  • The fee for the medical certificate needs to be paid on the day of application, except the medical certificate for liability purposes.
  • Cancellations and refunds are not available for completed medical certificates. However, a refund will be given in cases where the medical certificate cannot be prepared as a result of a decision by the doctor.

Receipt of medical certificates
Preparation period
  • About 2~3 weeks are required for preparation.
  • You will need to attend the hospital again if a medical examination is required to prepare the medical certificate
Notification after preparation of the medical certificate
  • You will be notified at the contact details provided in the application when the document is complete.
  • No phone call will be provided if you requested to receive the medical certificate by post.
Location and time of receipt
  • Please go to document reception on weekdays between 08:30 a.m. to 05:00 p.m.
  • Please be sure to bring your patient card with you when you come for collect it.

Information for those refusing to receive blood transfusions

  • We provide medical care services based on a relationship of mutual trust with patients and, therefore, make the best effort in implementing informed consent when providing medical care.
  • We make the best effort in providing treatment without blood transfusions or the minimal procedure necessary. However, in cases where patients who refuse blood transfusions for reasons of religious beliefs are in a condition where their lives are at risk without the blood transfusion, the blood transfusion will be performed according to medical ethics.
