A trial run of the "Online Insurance Eligibility Verification with My Number Card", which will be fully operational from October 2021, will be conducted in order to examine operational issues and verify data.
　
Patients who are willing to cooperate with us are requested to pay attention to the following points:

１　Please apply in advance for the use of your health insurance card at the
　　My Number Card website. (see URL below).

　　https://myna.go.jp/html/hokenshoriyou_top.html

２　The following functions will be supported by this hospital. During the trial period, 　
　　we will also check your health insurance card for data verification, so please bring 　
　　your health insurance card as well.
　（1）Confirmation of health insurance card for first visit.
　（2）Confirmation of health insurance card for return visit (and every first visit of the
　　　　month)

　３　Functions other than the above No.2 (e.g,certificate for ceiling-amount
　　　application) will not be supported during the trial operation.

　４　Trial operation may be suspended without notice due to system management or
　　　other reasons.

＜Procedure Flow＞
　For first visit

（1）Please ask to use your My Number Card at the reception for new patients.
（2）Please place your My Number card on the card reader and via face recognition
　　  or PIN number.
（3）Please submit your health insurance card at the reception for new patients.

　For return visit (or every first visit of the month)
（1）Please ask to use your My Number Card at the  A outpatient reception desk.
（2）Place your My Number card on the card reader at the A outpatient reception desk 　　  and via facial recognition or PIN number.
（3）Please submit your health insurance card at the outpatient counter that you are 
        going and also please inform the staff that you have done your My Number Card 
        verification.
